How Ukrainian communities sustained the HIV response during the war: a report by VirusOFF

Over the past three years, Ukraine has gone through an extraordinary crash test: despite war, humanitarian crises, and millions of displaced people, the country has not only preserved but also strengthened its HIV response system. Within the framework of an emergency aid project funded by ViiV Healthcare Positive Action, the platform VirusOFF presents the discussion paper “Lessons on Resilience: The Experience of Ukrainian Communities Affected by HIV During Full-Scale War” .

This report is more than just analysis — it is a roadmap for the world. At a time when humanity increasingly faces wars, epidemics, and crises, Ukraine’s experience shows: resilience is born where there is mutual support, trust, and readiness to take responsibility.

The paper summarizes Ukraine’s unique experience and demonstrates that, despite mass displacement and enormous humanitarian challenges, the country preserved the stability of its HIV and TB response systems. Moreover, the very communities often labeled as “vulnerable” or “marginalized” became leaders in providing humanitarian support and continued to fight for health and human rights.

People living with HIV, sex workers, LGBTQ+ communities, people who use drugs, and formerly incarcerated individuals did not remain passive victims of the war. On the contrary, they organized shelters, evacuated others, established systems to deliver medicines and food, and developed innovative solutions to support their peers. This experience proved that even under the most extreme conditions, continuity of HIV and TB treatment is possible. Through flexible networks, the adaptation of local initiatives, and partnerships with governmental and international actors, communities ensured access to therapy, prevention, and psychosocial support.

The report outlines eight key lessons from Ukraine’s experience. It shows that community-led groups have unique qualities that ensure resilience in emergencies. Preparedness for crises is essential for rapid response. Flexibility from donors and partners made it possible to quickly redirect resources to emerging needs. Meeting basic needs — from housing to food — proved just as important as medical services. Mental health cannot be overlooked, as it gives people the strength to continue the struggle. Networks and partnerships have proven vital, and the inclusion of community representatives in decision-making was critical for effectiveness. Finally, the report emphasizes that crises, despite their destructiveness, can also create opportunities for growth.
